Specialized Diabetes Program

Our specialized diabetes program is led by nurses and pharmacists who are certified diabetes educators. They can help you learn how to manage your blood sugar and live well with diabetes.

Your Primary Care Provider may refer you to the specialized Diabetes Program if you:

  • Recently found out that you have diabetes

  • Are starting to use insulin

  • Are changing the amount of insulin you use

  • Have low blood sugar at least once a week

  • Have complex medical conditions in addition to diabetes


What should I expect?

  • A Certified Diabetic Educator (registered nurse or pharmacist) will work with you, based on your personal goals and medical needs, to create an individualized plan of care.

  • Over 1-2 visits, education can include: blood sugar targets, medications (including insulin adjustments), diet and exercise recommendations, information on other diabetes-related services, and more.

  • You will receive your own copy of your plan of care and summary of your visit. The Certified Diabetic Educator will update your primary care provider.

How Do I Register for the Specialized Diabetes Program?

If you are a patient with the Toronto Western Family Health Team and interested in this program, ask your primary health care provider to refer you. A referral from your health care provider is required for the specialized diabetes program.
